Our office maintains rigorous control of our daily appointment schedules so that a person needing urgent urologic care can be seen as soon as possible, including the same day if your condition is deemed urgent by our staff.

Access Your Patient Portal
Prior to your scheduled appointment please access your Patient Portal. This will speed up your check in process at the office.
You will be given a username and password and will receive any updates made to your portal via your personal e-mail given.
Information obtainable through the portal will include:
- View labs results
- Send messages to our clinical staff
- Request prescriptions refills
- View billing statements
What to Bring to your Appointment
Please bring the following information with you for your appointment:
Patient Registration Form
Insurance card(s) and driver’s license
Physician referral forms, if required by your insurance, and the name of your referring physician
A list of your current medications, including over-the-counter, with dosage, frequency and start date, pharmacy name and phone number
Recent imaging reports/films/CD
Information about your medical and surgical history
At the Office
Be prepared to provide a urine sample. Also, for your protection, a photo will be taken to confirm your identity at future appointments.
Co-payments, Insurance and Billing
Co-payment and/or co-insurance will be collected on the day of service. For questions regarding insurance, please dial 480.897.2727 ext. 117 or ext. 119. All statements are generated bi-monthly and can also be accessible through the patient portal.
Please arrive 15 minutes prior to your scheduled appointment time.
