Our office maintains rigorous control of our daily appointment schedules so that a person needing urgent urologic care can be seen as soon as possible, including the same day if your condition is deemed urgent by our staff.

What to Bring to your Appointment
Please bring the following information with you for your appointment:
Patient Registration Form
Insurance card(s) and driver’s license
Physician referral forms, if required by your insurance, and the name of your referring physician
A list of your current medications, including over-the-counter, with dosage, frequency and start date
Recent imaging reports/films/CD
Information about your medical and surgical history
At the Office
Be prepared to provide a urine sample. Also, for your protection, a photo will be taken to confirm your identity at future appointments.
Co-payments, Insurance and Billing
Co-payment and/or co-insurance will be collected on the day of service. For questions regarding insurance, please dial 480.897.2727 ext. 117 for Dr. Worischeck or ext. 119 for
Dr. Grossklaus. All statements are generated bi-monthly.
